Bad Credit Agency

Does anyone have a template that can help me repair my credit report?

I am in the process of repairing my credit. I have a copy of my report from each agency, and I have support to show that many of these credit lines are inaccurate or incomplete. however, I would like to know how to format my response so that the reporting agency can understand my response more clearly. Any suggestions are appreciated:)
